A Private Criminal Complaint Form is used by private citizens requesting criminal charges filed against a defendant. A Private Criminal Complaint Form is required to be completed prior to meeting the Assistant District Attorney on Friday Mornings in District Court 05-2-02. INDIVIDUALS WISHING TO SPEAK WITH THE DISTRICT ATTORNEY MUST BRING THE FOLLOWING SIGNED FORM: When requesting a time payment plan/bond modification the defendant is required to fully complete and comply with this form. "Nunc Pro Tunc" is the act of filing an appeal late, beyond the 30 day limit, as if it were timely.
The Landlord-Tenant complaint form is solely for landlords. Tenants wishing to sue landlords must use the Civil Complaint Form.
